Community Transition Specialist

Role Overview

The Community Transition Specialist supports individuals with severe mental illness, often co-occurring substance use disorders and cognitive impairment, in their transition from custody to the community. This involves coordinating care and services to ensure a successful reintegration.

This role is part of a multidisciplinary team working collaboratively with various organizations to provide continuity of care. The specialist will utilize general care coordination and discipline-specific skills to link patients to relevant community services and support their initial reintegration into community care.

The team aims to develop mental health care plans, establish necessary therapeutic supports for patients, and provide follow-up community care to those leaving custody. A recovery-oriented approach promotes positive health outcomes and community participation.

Location

Services are based at Justice Health Olympic Park, located at 10 Herb Elliott Avenue, Sydney Olympic Park. Public transport and parking options are available, with easy access via M4 and Parramatta Road via Homebush Bay Drive. Bicycle paths surround the site, and bicycle storage and showers are available on-site.

Requirements
  • Current unrestricted Authority to Practice as a registered nurse or equivalent qualifications and experience for allied health professionals
  • Relevant post-registration qualifications in mental health and at least 3 years' experience working in mental health settings
