Clinical Mental Health Specialist - Community Transitions
Role OverviewThis is a dedicated position that supports Aboriginal and Torres Strait Islander people who experience severe mental illness to access the necessary care and support required to ensure a successful transition from custody to the community.
- This role involves working with multidisciplinary teams to provide collaborative approaches to ensuring continuity of care for patients.
The successful candidate will be responsible for developing mental health care plans and establishing necessary therapeutic and cultural supports for Aboriginal people with serious and enduring mental health needs as they transition out of custody and return to or reconnect with their community.
Key Responsibilities:
- To work collaboratively with other healthcare professionals and organisations to provide continuity of care and support for patients.
To be successful in this role, you will need to meet the following requirements:
- Be of Aboriginal or Torres Strait Islander descent.
- Hold a relevant degree or higher qualification.
- Have current registration with AHPRA or eligibility for membership of AASW.
- Have at least three years full time equivalent post registration experience in mental health.
