Job DescriptionWe are seeking a skilled and dedicated Community Registered Nurse to join our team. As a Community Registered Nurse, you will be responsible for providing advanced nursing practices in primary healthcare, driving the development, implementation, and evaluation of strategies and programs to achieve optimal health outcomes for our clients.
You will also provide palliative care to clients and support carers and families in community settings when entering the terminal phase.
Responsibilities- Provide clinical leadership and guidance to junior staff members
- Develop and implement individualized care plans to meet the unique needs of each client
- Collaborate with multidisciplinary teams to ensure comprehensive care delivery
- Participate in quality improvement initiatives to enhance service delivery and patient outcomes
To be successful in this role, you will need:
- A current registration as a nurse with the Nursing and Midwifery Board of Australia
- At least 3 years of post-registration experience in a relevant field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Capacity to travel to various locations within the region
