Our Aged Care Assessment Service (ACAS) plays a pivotal role in enhancing the lives of older people.

As an integral member of our multidisciplinary team, you will provide expert clinical leadership and undertake clinical assessment and delegation functions at one or more sites as an expert ACAS clinician.

The primary objective of the ACAS is to assess the needs of frail elderly clients and their carers in public and private hospitals, the community, and residential aged care facility settings within the Northern Sydney Local Health District.

As Clinical Team Leader, you will work closely with healthcare professionals, carers, and stakeholders to ensure seamless care delivery and data collection activities within the ACAS system.

Key responsibilities include:

  • Providing expert clinical guidance to clients, carers, ACAS Manager, and other stakeholders
  • Leading a multidisciplinary team and managing Key Performance Indicators
  • Collaborating with Commonwealth Home Support Program providers and community support service providers for comprehensive assessment and care planning
