**Job Description**

We are seeking both clinically qualified and non-clinically qualified Home Care Specialists to join our team. As a Home Care Specialist, you will conduct face-to-face assessments using the Integrated Assessment Tool and develop Individualised Support Plans tailored to client needs, preferences, and circumstances.

Responsibilities:

  • Conduct comprehensive home assessments to identify client needs and preferences
  • Develop and implement individualised care plans to meet client needs
  • Liaise with referral sources and community organisations to provide clients with appropriate support
  • Complete assessment-related documentation in a timely manner
  • Work independently while meeting key performance indicators

Requirements:**

  • Diploma or degree in social care, health services, psychology, counselling, or related fields
  • Experience in case management, community services, or social work
  • Aged care assessment experience, including home or risk assessments for vulnerable populations
