Intake Case Worker

We are seeking a highly skilled and compassionate Intake Case Worker to join our Supporting Recovery Local Care Team. As the first point of contact for clients who have experienced family and domestic violence, you will play a critical role in providing high-quality case management and referral pathways.
Key Responsibilities- Undertake initial assessments to identify client needs and develop tailored support plans
- Provide case management and referral services to clients, ensuring a trauma-informed approach
- Maintain accurate records and databases to track client progress and referrals
- Collaborate with multidisciplinary teams to ensure seamless service delivery
- Represent the organisation at inter-agency meetings and events
- Qualifications in Social Work, Social Science, or related field
- At least 2 years of case management experience, preferably in trauma-informed practice
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Strong understanding of trauma-informed practice and child protection issues
