Job Summary

We are seeking experienced Disability Support Workers to join our team in providing support and services to clients with disabilities in their own homes. As a Disability Support Worker, you will work closely with our clients to develop and implement personalized care plans, promoting their independence and well-being.

Key Responsibilities
  • Provide support and services to clients with disabilities in their own homes, promoting their independence and well-being.
  • Develop and implement personalized care plans in consultation with clients, their families, and other healthcare professionals.
  • Assist clients with daily living activities, such as personal care, meal preparation, and household management.
  • Encourage clients to participate in activities that promote their physical and emotional well-being.
  • Work collaboratively with other healthcare professionals to ensure comprehensive care and support.
  • Maintain accurate records of client interactions and care plans.
Requirements
  • Experience working with clients with disabilities in a community care setting.
  • Certificate III in Disability, Individual Support, or Aged Care.
  • Current Drivers Licence and Reliable Car.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
