Care Coordinator

We are seeking an experienced and motivated Care Coordinator to join our team at Right at Home Brisbane North and South. As a Care Coordinator, you will play a vital role in managing and coordinating services for our clients, ensuring they receive the highest level of care and support.
Key Responsibilities- Manage and coordinate services for NDIS and HCP participants, including arranging repairs, organizing meals, and scheduling appointments.
- Engage with stakeholders, including clients, family members, and healthcare professionals, to ensure seamless care delivery.
- Conduct regular assessments to identify client needs and develop personalized care plans.
- Build and maintain relationships with clients and their support networks.
- Collaborate with office staff to provide exceptional customer service and support.
- Document and maintain accurate records of client interactions and care plans.
- Cert III/IV in Aged Care or Disability qualification with 1 year's experience in a coordinating role, or RN with community nursing background and AHPRA registration.
- Experience in Aged & Disability care, with a strong understanding of NDIS and HCP principles.
- First Aid/CPR Certificate and Australian Driver's Licence.
- NDIS Workers Screening Card (P) and Blue Card (P).
- Criminal History Check.
- Excellent communication and interpersonal skills, with the ability to work in a team environment.
- Experience with Microsoft Word, Excel, and Outlook, with a strong understanding of administrative tasks.
- Ability to work under pressure and prioritize tasks effectively.
- Problem-solving capabilities and a customer-focused approach.
