Role Overview

We are in search of a Clinical Care Coordinator to guide our care team in collaboration with the Clinical Care Manager and General Manager. Your primary responsibility will be to ensure adherence to legislative and regulatory standards, enabling us to maintain our high care standards and promote a fulfilling lifestyle for our residents. The Clinical Care Coordinator will oversee the assessment and evaluation of individual resident care needs, providing essential support and direction to nursing staff and caregivers.

Key Responsibilities

  • Oversee all clinical services in accordance with company policies and procedures.
  • Promote continuous improvement initiatives across all clinical and lifestyle services.
  • Ensure accurate and timely completion of clinical documentation.
  • Coordinate and assist with scheduled audits and clinical performance indicators.
  • Maintain up-to-date and relevant resident health records and clinical documentation.
  • Adhere to and monitor best practice guidelines for medication management.

Candidate Profile

The ideal candidate will possess:

  • Relevant tertiary qualifications or equivalent recognized in Australia.
  • Current AHPRA registration as a registered nurse.
  • Significant experience in senior clinical roles within the health sector, particularly in aged care.
  • A solid understanding of care assessments in the aged care industry and familiarity with AN-ACC.
  • Excellent interpersonal and communication skills, both written and verbal.
  • Proficiency in various IT applications.
  • A comprehensive understanding of AN-ACC funding and aged care standards.
