You will provide high-quality personalized care services to participants and support them in achieving personal goals.
Your key responsibilities include:
- Supporting daily living activities and meeting personal care needs.
- Facilitating community access opportunities for participants.
- Maintaining accurate documentation to monitor and report on participant progress and healthcare.
- Implementing and reviewing participant Support Plans.
- Providing tailored supports that meet each participant's unique goals.
A successful Support Worker demonstrates a commitment to person-centered care and genuine enthusiasm for empowering NDIS participants.
Key qualifications include a Certificate III in Individual Support (preferably Disability or Ageing) and a valid NDIS Worker Check. You will also require relevant module completion.
