About the Role

We are seeking an experienced Senior Clinical Social Worker to join our team at St Vincent's Health Australia. As a senior member of our social work team, you will be responsible for providing high-quality clinical social work services to patients, families, and carers in the areas of aged care and rehabilitation.

Key Responsibilities
  • Deliver comprehensive bio-psychosocial assessments for patients in the allocated areas
  • Conduct risk assessments related to capacity and self-determination in aged care
  • Facilitate individual and family counseling to adapt to medical conditions and lifestyle changes
  • Participate in care and discharge planning, including referrals to community services and facilitation of respite and long-term accommodation
  • Assess matters of risk related to domestic and family violence and elder abuse
  • Advocate for patients and carers both inside and outside the hospital as required
  • Liaise with external organizations and agencies to facilitate the provision of social work services in the area of practice

Requirements

To be successful in this role, you will need:

  • A Bachelor of Social Work or equivalent tertiary qualification creating eligibility for membership of the Australian Association of Social Workers (AASW)
  • Demonstrated experience in clinical health social work settings with high-level skills in aged care social work service
  • High-level understanding and experience of complex social issues affecting patients, families, and carers in aged care services, rehabilitation, disability, and residential care
