St Vincent's Health Australia is seeking a Clinical Lead Social Worker to join our team in Sydney.

About the Role

The Clinical Lead Social Worker will be responsible for providing high-quality social work services to patients, families, and carers in designated areas of clinical practice. This role requires strong leadership skills, excellent communication, and problem-solving abilities.

Key Responsibilities
  • Provide comprehensive bio-psychosocial assessments for patients in allocated specialties
  • Conduct risk assessments related to capacity and self-determination in aged care
  • Facilitate individual and family counseling to adapt to medical conditions and lifestyle changes
  • Participate in care and discharge planning, including referrals to community services and respite and long-term accommodation
  • Assess matters of risk related to domestic and family violence and elder abuse
Requirements
  • Bachelor of Social Work or equivalent tertiary qualification creating eligibility for membership of the Australian Association of Social Workers (AASW)
  • Demonstrated experience in clinical health social work settings with high-level skills in aged care social work service
  • High-level understanding and experience of complex social issues for patients, families, and carers in regard to aged care services, rehabilitation, disability, and residential care

Salary

$80,000 - $110,000 per annum, depending on experience.
