Senior Clinical Social Worker

In this senior clinical social work position, you will provide comprehensive psychosocial services to patients, families, and carers in designated areas of clinical practice. As a seasoned professional, you will network with other social work providers, maintain standards of AASW professional code of ethics, and represent the social work department on internal and external committees.
Key Responsibilities:- Deliver high-quality social work services in designated areas of clinical practice.
- Conduct bio-psychosocial assessments for patients in allocated areas.
- Facilitate individual and family counselling, adaptation to medical conditions, and lifestyle changes.
- Participate in care and discharge planning, including referrals to community services.
- Assess risk matters related to capacity and self-determination in aged care.
- Bachelor of Social Work or equivalent qualification creating eligibility for membership of the Australian Association of Social Workers (AASW).
- Demonstrated experience in clinical health social work settings with high-level skills in aged care social work services.
- High-level understanding of complex social issues in aged care services, rehabilitation, disability, and residential care.
- A competitive salary of $85,000 - $105,000 per annum.
