Senior Patient Care Coordinator

We are seeking a skilled and dedicated Registered Nurse to provide quality clinical nursing services to doctors and patients.
Responsibilities:- Deliver high-quality clinical nursing services as part of the primary care team.
- Manage chronic disease programs and health assessments to ensure optimal patient outcomes.
- Perform triage, vaccinations, wound care and pathology collection with precision and care.
- Coordinate the treatment room to maintain a safe and efficient environment.
- Maintain accurate patient recall systems and follow-up as required.
- Minimum 1 year post-registration experience in a similar healthcare setting.
- RN AHPRA registration and a commitment to ongoing professional development.
- Strong communication and time management skills to ensure seamless collaboration with the team.
- A caring and compassionate approach to patient care that prioritizes empathy and understanding.
