Lead Aged Care Team

This leadership position is responsible for guiding a high-performing team through the aged care reforms, providing operational oversight and support to the Heidelberg assessment team.

The successful candidate will have well-developed leadership skills, a passion for aged care services, and a strong commitment to delivering high-quality assessment outcomes. They will also have a strong focus on quality and service improvement, and a proven ability to support teams through change.

  • Key Responsibilities
  • Provide operational oversight and support to the Heidelberg assessment team.
  • Support a positive team culture, oversee intake and triage functions, and ensure program achievement of key performance indicators.
  • Maintain assessment expertise and ensure timely response to referrals.

Essential Requirements

  • Current registration as a registered nurse or allied health professional, or eligible for membership of a relevant professional association.
  • Excellent interpersonal and communication skills that support team engagement, patient care, and collaboration.
  • A willingness to think outside the square and think creatively to achieve the best possible program and client outcomes.
  • Demonstrated flexibility to adapt with program and organisational requirements.
