Job Description:

As an Aged Care Navigator, you will play a vital role in enriching the lives of older Australians by guiding them through their aged care journey. Your expertise and empathy will ensure that every individual receives the best possible support tailored to their unique needs and goals.

  • Conduct thorough assessments to identify care requirements and develop personalized plans.
  • Collaborate with clients, families, and care teams to deliver exceptional customer service and achieve seamless transitions from enquiry to care delivery.
  • Maintain accurate documentation and ensure compliance with regulatory standards.
Requirements:

To succeed in this role, you will need:

  • A compassionate and detail-oriented approach to case management.
  • Experience in aged care, with a focus on empowering older Australians.
  • Strong communication and interpersonal skills, with the ability to build relationships with clients, families, and care teams.
  • A diploma in Service Coordination, Community Services, or equivalent qualification.
  • Excellent technology skills, including proficiency in CRMs and digital tools.
