**Job Title:** Home Support Coordinator

We are seeking a dedicated and compassionate Home Support Coordinator to join our team. As a key member of our Housing First program, you will play a vital role in supporting individuals and families to achieve stable and secure long-term housing.

The ideal candidate will have a clear understanding of the Housing First principles and be able to provide effective support planning for each individual or family member. This may include conducting thorough needs assessments, providing practical support where necessary, and connecting individuals with additional support services.

You will work collaboratively with our multidisciplinary team to ensure that clients receive holistic support and achieve their housing goals. This is an excellent opportunity for someone who is passionate about making a positive impact in the lives of others.

About this role:

  • To assist individuals and families in obtaining and maintaining safe and stable long-term housing.
  • To conduct thorough needs assessments and develop effective support plans.
  • To provide practical support where necessary, including connection with additional support services.
  • To work collaboratively with the Homes First multidisciplinary team.
  • To maintain accurate records and reporting.
