Disability Referral Specialist

This is a highly impactful role where you will be working closely with the Centre for Disability Employment Research and Practice team to connect individuals with disabilities to meaningful employment opportunities.
Main Responsibilities- Establish, develop and maintain strong referral partnerships with Plan Managers and Support Coordinators
- Effectively communicate Work First's service scope and objectives
- Support new clients and families in understanding service agreements and onboarding processes
- Liaise with administration to ensure contracts and scheduling are accurately established
- Provide timely and accurate updates to key stakeholders regarding NDIS and program changes
- Maintain a Weekly Work Plan via Microsoft Teams
- Collaborate with team members to ensure continuity of service during periods of staff change
- Uphold confidentiality and professional conduct across all interactions
Success in this role will be evident when:
- A consistent stream of referrals is maintained (set monthly KPIs)
- Clear communication with referrers and families is demonstrated
- Accurate onboarding that reflects our values of long-term, person-centred employment support is achieved
- Demonstrated excellent interpersonal and relationship-building skills
- In-depth understanding of the NDIS, particularly Core and Capacity Building funding
- Ability to manage multiple priorities with strong attention to detail
- Tech-savvy - proficient in using Microsoft Teams, email, shared files and client management systems
- Previous experience of 3+ years in disability, allied health, employment or NDIS-related roles is highly regarded
- Police Check and Working with Children Check required
