
Empathetic Family Support Specialist

We are seeking a highly skilled and compassionate Family Preservation Caseworker to join our team. As a key member of our Outreach Youth Hope Program, you will work closely with families and young people to provide support and guidance in maintaining connections within the community.
About the Role
The successful candidate will be responsible for collaborating with families and young people to develop and implement case plans that cater to their unique needs and goals. This may involve working with clients to identify and address barriers to progress, providing resources and referrals as needed, and facilitating communication between family members and external services.
Key Responsibilities
- Develop and maintain effective relationships with clients and their families
- Conduct thorough risk assessments and case planning meetings to ensure client needs are met
- Prioritize tasks and manage workload to meet deadlines and achieve desired outcomes
- Communicate regularly with colleagues and stakeholders to share knowledge and best practices
- Maintain accurate and up-to-date records of client interactions and progress
Requirements
- Tertiary qualification in Social Work or related field
- Minimum 2 years' experience working with children and young people
- Strong understanding of Needs-Based Restorative Framework concepts and strategies
- Excellent communication and interpersonal skills
