About the Role

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Nurse Care Specialist. In this role, you will have the opportunity to make a genuine impact within your local community by delivering professional in-home nursing care.

Key Responsibilities
  • Travel between customers' homes and provide individualized and personalized support to customers in accordance with their support plans and/or medical practitioner's directions.
  • Work under the guidance and supervision of medical practitioners, collaborate with allied health professionals, monitor and communicate the customer's condition, and other health and wellness-related activities.
  • Demonstrate a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with our company's policy and procedures, and legislative requirements.
  • Administer medication and provide complex clinical care, including palliative care, bowel care, catheter care, stoma care, oxygen therapy, and more.
  • Monitor and communicate on the customer's well-being and report on progress of customers.
  • Ensure that all services are provided in a caring and respectful way in accordance with our company's policy and procedures, legislative requirements, and program-specific guidelines.
  • Provide training, coaching, and guidance to Community Support Professionals, Enrolled Nurses, and Support Staff/Case Managers as required.
  • Conduct skills assessments of Community Support Professionals performing complex care services as required.
  • Demonstrate a commitment to best practice and evidence of continued professional development.
  • Participate in staff meetings and attend training opportunities when required.
Requirements
  • Demonstrated high-level verbal and written communication skills.
  • Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation, and problem-solving skills.
  • Ability to work as part of a team.
  • Comfortable using technology (i.e., mobile devices) for accessing rosters and recording customer assessments and documenting feedback.
  • Current Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Evidence of continued professional development.
  • NDIS Worker Screening Clearance/Criminal Record Check.
  • Current CPR/First Aid Certificate.
  • Current Australian Driver's Licence.
  • Reliable, registered, and insured vehicle (comprehensive insurance is preferred).
  • Previous experience in the community care or health/aged care sector (preferred).
  • Proof of Vaccination (where relevant) i.e., COVID-19 vaccination certificate.
