Rehabilitation Care Specialist

We are seeking a skilled and compassionate Rehabilitation Care Specialist to join our multidisciplinary team. As a key member of our rehabilitation team, you will provide high-quality patient care and support patients in regaining independence and returning to their daily lives.

Key Responsibilities:

  • Delivering exceptional patient experiences while achieving high-quality clinical outcomes
  • Assisting patients to access supported services to maximise their physical gains and return to a rewarding lifestyle
  • Working with multidisciplinary teams to plan the care and treatment for patients through regular case conferences
  • Maintaining and promoting patient safety within our satellite unit through early intervention and escalation of care

Requirements:

  • Current nursing registration with AHPRA
  • Contemporary knowledge of best practice in acute, aged, or rehabilitation nursing
  • Experience as a registered nurse with a passion for patient care and achieving high-quality clinical outcomes, either from a rehab or acute setting
  • Strong collaboration skills to work effectively with team members to support the delivery of rehabilitation care
  • Confident communication skills to build rapport with staff, patients, and families
