We are seeking a highly motivated and experienced Registered Nurse to join our Transitional Aged Care Program (TACP) team in providing comprehensive assessment, care planning, and case management of older people in their homes.

The TACP Registered Nurse works as part of a multi-disciplinary community team to deliver individualised care plans that promote client independence and autonomy. The successful candidate will have the opportunity to make a meaningful difference in the lives of our clients and contribute to improving rehabilitation outcomes.

About the Role:
  • Conduct nursing assessments in clients' homes and liaise with other treating members of the allied health team.
  • Provide holistic quality nursing care in accordance with the Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies, NSW Health, and South Eastern Sydney LHD policies and procedures.
  • Assess, plan, organise, evaluate, and deliver individualised care plans that promote client independence and autonomy.
  • Collaborate with healthcare professionals and support services to ensure seamless transitions between hospital and community settings.
Key Requirements:
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/Midwife.
  • Demonstrated high-level written and verbal communication, interpersonal skills, and ability to identify key messages, issues, and concerns when communicating with others.
  • Ability to work autonomously and as part of a multi-disciplinary team, applying professional and ethical boundaries when dealing with complex situations.
  • Recent experience as a Registered Nurse in the area of aged care and/or rehabilitation, with current professional practice knowledge related to working in the community as a Registered Nurse.
