About the Role:- Conduct nursing assessments in clients' homes and liaise with other treating members of the allied health team.
- Provide holistic quality nursing care in accordance with the Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies, NSW Health, and South Eastern Sydney LHD policies and procedures.
- Assess, plan, organise, evaluate, and deliver individualised care plans that promote client independence and autonomy.
- Collaborate with healthcare professionals and support services to ensure seamless transitions between hospital and community settings.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/Midwife.
- Demonstrated high-level written and verbal communication, interpersonal skills, and ability to identify key messages, issues, and concerns when communicating with others.
- Ability to work autonomously and as part of a multi-disciplinary team, applying professional and ethical boundaries when dealing with complex situations.
- Recent experience as a Registered Nurse in the area of aged care and/or rehabilitation, with current professional practice knowledge related to working in the community as a Registered Nurse.
