Are you passionate about making a meaningful difference in people's lives?

We have an exciting opportunity for a dynamic and empathetic Compassionate Care Professional to join our team.

Your role is vital to empower clients and their families through individualised care planning and goal setting focused on well-being.

A typical day will involve:

  • Home visiting clients in the community who have been referred for transitional aged care program case management and services at home after a hospital stay
  • Participating in the development, implementation, and evaluation of services and programs that meet clients' unique needs
  • Building relationships with families and carers that acknowledge their requirements
  • Collaborating with clients to set realistic and achievable goals
  • Implementation of best practice and evidence-based approaches for care, including working closely with a multidisciplinary team

You will need to be comfortable working independently in the community, possess excellent verbal communication skills, and demonstrate an empathetic, patient, and caring nature.

This role requires a Bachelor's Degree or Diploma in Community Services (Service Coordination) or equivalent knowledge and experience, as well as a valid Australian driver's licence and intermediate level of computer skills.
