Community Care Coordinator

Overview:

Right at Home Central QLD is a leading provider of in-home care services dedicated to delivering compassionate and respectful care to individuals across the Wide Bay Region of Queensland. We are seeking an Enrolled Nurse to join our team, working closely with clients and their families to provide personalized support and guidance.

As an Enrolled Nurse, you will be responsible for conducting comprehensive assessments, developing individualized care plans, and providing nursing and in-home care services. Your exceptional communication and interpersonal skills will enable you to build strong relationships with clients and their families, ensuring high-quality care delivery.

The ideal candidate will possess strong organizational and time management skills, as well as a commitment to continuous professional development. They will demonstrate empathy, client-focused care, and respect for others.

Responsibilities:
  • Engage with clients and their families to assess their needs and establish trusting relationships.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Provide nursing and in-home care services, including wound care, insulin administration, and medication management.
  • Address inquiries and provide detailed feedback as required.
Qualifications:
  • Enrolled Nurse (EN) with 1-2 years of experience in community nursing and current AHPRA registration.
  • Alternatively, a Certificate III or IV in Aged/Disability Care with a minimum of 4 years' experience.
  • Proven experience in aged care, disability, or mental health within a community setting.
  • Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
