Job Summary

We are seeking a highly skilled Occupational Therapist to provide expert services to our valued customers in the Rockhampton area. As a passionate advocate for community age care, you will play a vital role in delivering high-quality OT services that support our customers in achieving their goals related to daily living, mobility, and community participation.

The Role

This exciting opportunity involves managing a diverse range of customer presentations across various aged care funding areas, including Home Care Packages, Commonwealth Home Support Program, and Short-Term Restorative Care. You will be responsible for conducting comprehensive assessments, developing tailored therapy plans, and collaborating with clients, families, carers, GP's, and other professionals to ensure holistic care.

Key Responsibilities
  • Conduct thorough assessments and create personalized therapy plans to meet the unique needs of our customers.
  • Provide expertise in the provision of simple and complex equipment prescription, as well as major and minor home modifications.
  • Collaborate effectively with clients, families, carers, GP's, and other healthcare professionals to deliver seamless and comprehensive care.
Requirements
  • Hold a tertiary qualification in Occupational Therapy and maintain current registration with AHPRA.
  • Demonstrate significant practical experience or extensive knowledge in providing OT Services, preferably in a Rehabilitation and/or Aged Care setting.
  • Possess excellent interpersonal, communication, organization, and time management skills.
