Care Coordinator Role

This is a Permanent Full-Time position that involves managing referrals, onboarding new clients, and adapting care plans to meet changing needs. Effective care coordination requires strong interpersonal skills, the ability to work independently, and excellent time management skills.

You will coordinate home visits and work closely with clients and their families to deliver person-centred support. Our goal is to ensure clients feel supported and valued while contributing to the ongoing growth of our aged care programs.

Key Responsibilities:
  • Comprehensive Client Assessments

Undertake comprehensive client assessments to develop client profiles and care plans in collaboration with the client/representative and other relevant stakeholders. This includes assessing client needs, developing individualized care plans, and coordinating necessary resources.

Care Plan Monitoring and Review

Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits. This ensures that clients receive timely and effective support, and that their needs are met in a holistic and coordinated manner.

Client Safety and Risk Management

Monitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clients. This may involve working with internal stakeholders to develop and implement strategies to mitigate risk, and collaborating with external agencies to provide additional support as needed.

Stakeholder Collaboration

Collaborate effectively with internal stakeholders to ensure clients' needs are met. This includes communicating regularly with colleagues, developing and implementing care plans, and providing updates on client progress.

Community Representation

Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community. This involves building relationships with community partners, presenting information about our services, and engaging with local communities to promote our mission and values.
