As a key member of our clinical team, you will play a vital role in providing holistic and flexible nursing services to individuals living in the community.
Job Description:- Provide preventative based approach to healthcare through clinical skills and knowledge.
- Meet with customers in their own homes to assess their needs and implement care plans.
- Conduct regular home visits to monitor customer progress and provide ongoing support.
- Maintain accurate and up-to-date customer records with supporting documentation.
- Work collaboratively as part of a multidisciplinary team to achieve optimal customer outcomes.
- Tertiary qualification in Nursing and current registration with AHPRA.
- Experience working within the health, community, or social service sector.
- Demonstrated ability to work efficiently in an autonomous capacity.
- Excellent communication and time management skills.
- High level of computer literacy and effective data management.
