Community Care Specialist Role

We are seeking a compassionate and skilled Community Care Specialist to join our team, providing high-quality care in a community setting.

About the Position
  • This is a full-time position involving both office-based and fieldwork responsibilities.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to connect with clients and their families.

Key Responsibilities:
  • Engage with clients to assess their needs and establish trusting relationships.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Provide nursing and in-home care services, including wound care and medication management.
  • Address inquiries and provide detailed feedback as required.
Requirements:
  • Enrolled Nurse (EN) with 1-2 years of experience in community nursing and current AHPRA registration.
  • Alternatively, a Certificate III or IV in Aged/Disability Care with a minimum of 4 years' experience.
