**Role Overview**:

This role plays a pivotal part in coordinating the intake system, effectively assessing and meeting the diverse needs of families and individuals through comprehensive case management.

**Key Responsibilities**:

- Conduct thorough screenings and assessments to identify support requirements for clients referred to social health services.
- Deliver high-quality case management plans tailored to individual client needs, incorporating a wide range of assessed social and wellbeing supports.

**Required Skills and Qualifications**:

- Possession of a Bachelor qualification in a relevant field such as Early Childhood, Social Work, Human Services, Behavioural Science or equivalent.

- Demonstrated ability to work collaboratively with diverse people and presentations, providing case managed social work driven supports.

- Accurate and consistent record-keeping skills, with experience in recording case notes.

- Ability to operate with minimal direction, communicate effectively within multidisciplinary teams, and contribute to innovative service development.

- Willingness to travel across various service sites and engage with clients outside traditional clinic settings.

- Requirements include holding a Working with Children's Card, NDIS Working Screening Card, National Police Clearance, and a valid Driver's Licence.

**Preferred Qualifications**:

- Previous experience working within multi-disciplinary teams.

- Experience working in Aboriginal Medical Services would be highly regarded.
