Surgical Access Patient Care Coordinator

Purpose: This role is a critical component of our Surgical Access Office. We are seeking an experienced and compassionate Patient Care Coordinator to join our team. The successful candidate will have excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve in a dynamic environment.
Key Responsibilities:
- Act as a patient advocate, providing education and support during the pre-operative period.
- Take a lead role in coordinating care provided by the multidisciplinary team to ensure the best possible outcome for every patient on the planned surgery list.
- Continually strive to improve the service we provide to both our patients and the health service, adhering to Planned Surgery Access Policy.
Required Skills and Qualifications:
- Experience and knowledge of hospital Patient Administration Systems (iPM).
- Advanced Microsoft Office Literacy.
- Well-developed communication and interpersonal skills with strong patient focus.
- Ability to work independently and as part of a multidisciplinary team.
