Job Overview

We are seeking a highly skilled and experienced Care Manager to lead our team in providing exceptional care to residents at Gracehaven Residential Aged Care Service in Bundaberg. As the key clinician, you will play a pivotal role in shaping the delivery of care services, ensuring high-quality outcomes for our clients.


Key Responsibilities
  1. Develop and implement consultative collaborative approaches to resident care planning, focusing on quality management activities;
  2. Evaluate aged care funding instrument data for accuracy relevance before submission, guaranteeing compliance with regulatory requirements;
  3. Maintain review nursing practice procedures clinical programs initiatives, fostering a culture of excellence within the service;
  4. Supervise report resident/client documentation, adhering to electronic documentation organizational requirements;
  5. C Coordinate provision of care by external service providers, ensuring seamless integration into our care pathways;
  6. Maintain monitor delivery of clinical care by direct care staff, upholding professional guidelines and standards.

Required Skills and Qualifications
  • Current registration as a registered nurse with AHPRACurrent experience managing leading clinical care in the aged care sector;
  • At least 5 years post-graduate experience in aged care acute care clinical settings;
  • Strong knowledge of the aged care accreditation system compliance, including AN-ACC;
  • Demonstrated ability to educate mentor motivate staff, promoting a positive work environment;
  • Superior communication skills, both written verbal;
  • National police NDIS screening certificate.
