We are seeking a Registered Nurse to fill the role of Care Coordinator in our Bundaberg facility.
Duties and Responsibilities- Coordinating and liaising with external providers for seamless care delivery.
- Overseeing and coordinating baseline and focus assessments to inform individualized care plans.
- Developing and reviewing care plans tailored to the unique needs and goals of each resident.
- Collecting accurate clinical and care data for timely submission and reporting, ensuring compliance with regulatory requirements.
- Providing clinical leadership and support to our Registered Nurses, fostering their growth and development in clinical leadership skills.
- Collaborating with the Clinical Manager to ensure quality clinical care is maintained, and overseeing follow-up activities as needed.
To be considered for this role, you will need:
- A current AHPRA registration as a Registered Nurse (RN).
- Minimum 3 years of clinical experience in the aged care sector.
- A valid driver's license and current Police Check and Blue Card.
- Australian Work Rights – sponsorship is not currently available.
- Post-graduate qualifications in Geriatrics, Dementia, or other aged care specializations.
- Current First Aid and CPR Certificate, or willingness to obtain one.
- Current NDIS check, or willingness to obtain one.
