A skilled and compassionate Registered Nurse is required to coordinate care for residents at a residential aged care home in Bundaberg.
Key Responsibilities- Liaise with external providers to ensure high-quality patient care.
- Coordinate baseline and focus assessments for residents, ensuring accurate clinical data is collected and submitted on time.
- Develop and review individualized care plans tailored to meet the unique needs of each resident.
- Provide leadership and mentorship to fellow RNs, promoting excellence in clinical practice.
- Assist the Clinical Manager in quality assurance and oversight.
- Passionate about aged care with a commitment to resident safety and support.
- Current AHPRA registration as a Registered Nurse (RN).
- Minimum 3 years' experience in aged care sector.
- Valid driver's license and willingness to obtain Police Check and Blue Card.
- Hold Australian Work Rights.
- Post-graduate qualifications in Geriatrics or Aged Care specialisation.
- Current First Aid and CPR Certificate.
- NDIS check.
