Care Professional Advocate

The role of a Care Professional Advocate involves working collaboratively with individuals to ensure they receive the best possible care and assistance.
To be successful in this position, you will need to possess a comprehensive understanding of NDIS support items and be able to assess, develop, and implement client support plans.
Key responsibilities include:
- Assessing clients' needs and developing individualized support plans that meet their requirements;
- Documenting all client interactions and progress against NDIS goals;
- Observing clients' physical and psychological status and reporting changes to the Service Manager;
- Providing one-on-one or group support to help clients overcome social and emotional challenges and make informed decisions about their care.
The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.
