Deliver exceptional patient care with our Nurse Navigation Service, a state-wide model of holistic support for individuals with complex health conditions. Our integrated approach ensures patients receive the most appropriate service when and where they need it. As a Nurse Navigator, you will work across system boundaries to actively advocate and improve health literacy, partnering with patients, families, and other healthcare services to enhance the patient's healthcare experience and quality of life.
The Nurse Navigator role employs advanced clinical and professional practice capabilities to deliver a positive patient-centred health experience. This involves coordinating patient care, reducing service fragmentation, educating, and empowering individuals. You will demonstrate capabilities that support the effective coordination of patients with complex care needs across all specialty services, focusing on those with multiple co-morbidities and high-frequency presenters for unscheduled care.
