We are seeking Aboriginal Health Workers to join our Palliative Care and Cancer Services Team. The team provides culturally responsive care and support for Aboriginal patients and their families.

The role involves supporting Aboriginal patients accessing SWSLHD Palliative Care and Cancer Services, ensuring their care is culturally responsive. As part of the team, you will work in collaboration with multidisciplinary teams to meet the clinical, cultural and psychosocial needs of Aboriginal patients.

To be successful in this role, you will require a Cert IV Aboriginal & Torres Strait Islander Health Care or other relevant qualification, and/or relevant experience working within Aboriginal Health services. You must also identify as an Aboriginal person and be accepted in the community as such.

Key responsibilities include:

  • Providing culturally responsive care coordination and case management services to Aboriginal patients and their families.
  • Establishing and maintaining linkages with internal and external service providers.
  • Working effectively as part of a multidisciplinary team to provide holistic and coordinated care.

This is an Identified Aboriginal/Torres Strait Islander Position. Exemption is claimed under Section 14 of the Anti-Discrimination Act 1977 (NSW).
