Job Description:

The role of the Advanced Case Manager is a pivotal position within our organization, focusing on managing complex and high-risk offenders who have completed part of their sentence in custody. As an integral member of our team, you will utilise evidence-based frameworks to deliver dynamic practice in the case management of offenders with complex needs and those who have committed serious offences.

Your responsibilities will include ensuring effective assessment, planning, intervention and review, as well as motivating and influencing individuals in the case management process, modelling pro-social behaviours and drawing on offender strengths based approaches.

You will also contribute to a strengthened and integrated through-care approach for offenders transitioning from prison onto court orders, through close partnerships with internal and external stakeholders.

Additionally, you will be responsible for encouraging offenders to take ownership, accountability and responsibility to organise necessary appointments with other services required, such as Centrelink, housing, and medical appointments, linking them with services including alcohol and other drugs (AOD), mental health, housing, and mental health.

Finally, you will produce written reports with appropriate analysis and recommendations to senior stakeholders and other standard documents in line with procedural guidelines.
