Job Role Overview

We are seeking an experienced professional to fill the role of Advanced Case Manager. As part of our team, you will play a critical role in helping support community safety and managing complex cases.

About the Job

The Advanced Case Manager is responsible for delivering dynamic practice in case management for offenders with complex needs. You will manage a caseload of high-risk offenders who have completed part of their sentence in custody.

  • Contribute to a strengthened and integrated approach for offenders transitioning from prison to community-based orders.
  • Work collaboratively with other services, including alcohol and other drugs (AOD), mental health, housing, and medical appointments.
  • Develop written reports with recommendations for senior stakeholders and other standard documents.
Key Responsibilities
  • Manage a caseload of complex offenders, providing individualised support and guidance.
  • Develop and implement strategies to reduce reoffending and improve community safety.
  • Collaborate with other agencies and services to provide comprehensive support to offenders.
