The RoleWe're seeking vibrant and motivated professionals to join our team as Disability Support Coordinators. In this position, you'll work closely with senior team members to establish relationships and support client goals alongside all stakeholders involved.
As a Disability Support Coordinator, your responsibilities will include:
- Liaising with direct clients, case managers, and all stakeholders who require our services: in-home supports, supported independent living, community access and participation, respite, etc.
- Assisting senior team members in end-to-end onboarding and ongoing monitoring for all participants in your allocated caseload.
- Collaborating with coordination, recruitment, and management to ensure best outcomes for all participants.
- Assisting in onboarding of new participants with all relevant compliance - including participant profiles, care plans, service agreements, schedules of supports, collating health assessments, etc.
- Ongoing face-to-face meetings with participants to ensure service standards are upheld.
To be successful in this role, you'll need:
- Minimum 1 year's experience working in disability from a client liaison perspective. E.g., case management, team leaders, etc.
- Exceptional attention to detail.
- Person-centred focus in all communications and duties.
- Ability to work autonomously and as part of a team.
- NDIS worker screening check or working with children check and police check.
- Excellent written and verbal communication skills.
- Experience meeting deadlines and working within a fast-paced environment.
- Driver's license and reliable vehicle.
