Health Outreach Specialist

The Health Outreach Specialist will be part of a multidisciplinary team delivering health provision to marginalized and vulnerable adults in Suffolk. The team's mission is to engage with hard-to-reach clients, reconnect those who have fallen out of mainstream services back to health and social care interventions.

Key Responsibilities:
  • Manage client cases, assess needs, and deliver outreach services to high-risk groups
  • Liaise and work jointly with partners serving asylum seekers, refugees, black and minority ethnic groups, ex-offenders, homeless individuals, and migrant workers
  • Carry out clinical duties assigned by the Clinical Team Manager

The team operates within the framework of NICE guidelines and evidence-based best practice, providing a flexible, accessible, and non-stigmatising service to marginalized and vulnerable adults.

Requirements:
  • Experience in managing complex client cases
