Healthcare Outreach Specialist

The Healthcare Outreach Specialist will be part of a multidisciplinary team providing essential health services to vulnerable adults in Suffolk. The role involves delivering targeted support and interventions to hard-to-reach groups, ensuring they receive timely access to healthcare resources.

Key Responsibilities:
  • Manage client cases, assess needs, and identify potential risks.
  • Collaborate with external partners serving diverse community groups, including asylum seekers, refugees, homeless individuals, and migrant workers.
  • Deliver outreach services, carrying out clinical duties assigned by the Clinical Team Manager.
Role Requirements:
