We are seeking a skilled and motivated Community Nurse to play a key role in providing high-quality patient care within Integrated Neighbourhood Teams.
Key Responsibilities:- Work collaboratively with other community nursing, therapy, and social care colleagues to provide holistic care to patients in their own homes.
- Deliver diverse caseloads that promote patient safety and well-being, working alongside experienced clinicians.
- Have the opportunity to work with a wide range of professionals, including Generic Support Workers, Physiotherapists, Occupational Therapists, social workers, independence wellbeing practitioners, and Matron.
