Integrated Community Services Opportunity

This role presents an exciting chance to be part of a dynamic team delivering community nursing services in Ipswich and East Suffolk.

  • As a Community Nurse, you will play a vital role in providing high-quality patient care within the comforts of patients' own homes.
  • You will work closely with other healthcare professionals, including Generic Support Workers, Physiotherapists, Occupational Therapists, social workers, independence wellbeing practitioners, and social prescribers.

The Integrated Neighbourhood Teams cover both rural and urban areas, providing holistic and sometimes complex nursing care. You will have the opportunity to develop your skills and knowledge while working with experienced teams and multidisciplinary teams.

For this role, we are looking for someone who is committed to teamwork, delivering high-quality patient care, and has a desire for ongoing professional development.

Key aspects of this role include:

  • Providing patient-centered care in a community setting
  • Working collaboratively with multidisciplinary teams to achieve optimal patient outcomes
  • Developing and maintaining strong relationships with patients, families, and carers
  • Participating in regular professional development and training opportunities
