Job Opportunity: Clinical Care Specialist

We are seeking a dedicated and compassionate clinical care specialist to join our team in the Hunter region. The ideal candidate will possess excellent clinical skills, strong communication abilities, and a passion for delivering high-quality patient care.

  • Provide expert clinical guidance and support to patients and caregivers
  • Develop and implement effective care plans that meet individual needs and goals
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Conduct comprehensive assessments and identify areas for improvement
  • Stay up-to-date with industry developments and best practices

About You:

  • A registered nurse with current registration under AHPRA
  • Current unrestricted drivers' license
  • NDIS Worker screening clearance
  • CPR Certificate (ongoing requirement)
  • National Police Check
  • Australian work rights
  • Strong knowledge of age-related healthcare and enablement principles
  • Excellent case management and coordination skills
  • Ability to think critically and solve complex problems
  • Strong written and verbal communication skills
  • Experience working with people who have dementia and their carers
