Job Role Summary:

This role is a case management specialist that provides support to adult and child victim-survivors of family violence.

  • Key responsibilities include providing trauma-informed, client-centred care in accordance with relevant standards and models of practice.
  • BUILDING TRUST-BASED RELATIONSHIPS WITH CLIENTS IS ESSENTIAL TO ENCOURAGING EMPOWERMENT, SAFETY AND PARTICIPATION.
  • CONDUCT INITIAL AND ONGOING ASSESSMENTS OF CLIENT NEEDS, RISKS AND GOALS FROM INTAKE TO EXIT.
  • DEVELOP AND IMPLEMENT INDIVIDUALISED CASE PLANS IN COLLABORATION WITH CLIENTS, ENSURING THEIR ACTIVE PARTICIPATION.
  • FACILITATE ACCESS TO APPROPRIATE SUPPORT SERVICES, COMMUNITY RESOURCES AND ADVOCATE ON BEHALF OF CLIENTS WHERE NEEDED.
About The Role:

As a Specialist Family Violence Practitioner, you will be working in a hybrid role providing place-based case management support across both refuge and outreach services.

This role requires strong communication and interpersonal skills to build relationships with clients, colleagues, and external stakeholders.

You will need to have excellent assessment and planning skills, as well as the ability to work autonomously and as part of a team.

The ideal candidate will have experience in working with vulnerable populations and possess knowledge of relevant policies and procedures.
