**Job Title:** Aged Care Clinical Assessor

**Job Summary:

Aged care clinical assessors play a vital role in evaluating the needs of elderly clients to determine suitable care and support services. We are seeking experienced professionals to join our single aged care assessment team.

**Key Responsibilities:

  • Evaluate the needs of elderly clients to determine appropriate care and support services
  • Conduct home support assessments using the Integrated Assessment Tool (IAT)
  • Develop and finalize personalized care plans addressing the holistic needs of clients

**Required Skills and Qualifications:

To be successful in this role, you will need:

  • Tertiary qualifications in a health-related discipline
  • Current Australian Health Practitioner Regulation Agency (AHPRA) registration
  • Minimum one year case management experience in assessment and goal-oriented support planning in aged, community, health
