About the Role


Australian Unity's Home Health platform is proud to partner with Ramsay Connect on the Diabetes Using Our Strengths Service (DUOSS) pilot research program.


This innovative program combines face-to-face and virtual care to empower Aboriginal and Torres Strait Islander people to manage their type 2 diabetes.


As our Aboriginal Peer Health Advocate, you'll play a vital role in this journey, supporting Aboriginal and Torres Strait Islander patients to engage in DUOSS and working closely with our clinical team to deliver compassionate, meaningful care.



Key Responsibilities



  • Provide respectful, culturally safe support to Aboriginal and Torres Strait Islander patients living with diabetes
  • Facilitate regular yarns by phone, videocall, or face-to-face in patients' homes
  • Organise activities that support patients' health and well-being
  • Liaise with clinical staff about any concerns and patient needs
  • Help patients with technology for health monitoring and communication


About You


You will engage with team members and customers virtually (phone, email, video) or face to face, using your lived experience with diabetes to provide holistic support to patients enrolled in the service.


As the Aboriginal Peer Health Advocate, you will require a strong understanding of cultural safety, with an ability to support Aboriginal and Torres Strait Islander patients compassionately.


Your expertise and qualifications will include:



  • A demonstrated ability to communicate effectively and sensitively within a diverse team
  • A strong understanding of cultural safety, with an ability to support Aboriginal and Torres Strait Islander patients compassionately
  • Willingness to travel for face-to-face appointments on Bundjalung Country
  • Confidence in using and troubleshooting patient apps and health monitoring devices
  • Knowledge of privacy and confidentiality practices in a digital health setting
