Clinical Care Coordinator

About the Role
Australian Unity is seeking a skilled Clinical Care Coordinator to join our team in delivering the Diabetes Using Our Strengths Service (DUOSS) pilot research program. As a key member of our multidisciplinary team, you will work closely with our Diabetes Educator and Aboriginal Health Worker to provide high-quality, culturally appropriate care to patients with type 2 diabetes.
Key Responsibilities
- Deliver technology-enabled, culturally sensitive care to patients, including face-to-face and remote patient monitoring
- Develop and implement evidence-based chronic disease management plans
- Coordinate care and provide health coaching to patients
- Clinically escalate any concerning results, signs, or symptoms
Requirements
- Registered Nurse with unrestricted AHPRA registration and community nursing experience
- Clinical skills in chronic disease management and delivering care within a multi-disciplinary framework
- Demonstrated ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people
