About the Role

Australian Unity is proud to partner with Ramsay Connect to deliver the Diabetes Using Our Strengths Service (DUOSS) pilot research program. This innovative virtual and face-to-face service incorporates remote patient monitoring, optimized management of diabetes medicines, education, and group discussions to help Aboriginal and Torres Strait Islander peoples manage their type 2 diabetes.

As the Aboriginal Healthcare Practitioner, you will lead the clinical delivery of the Diabetes Using Our Strengths Service (DUOSS). You will work in a digitally enabled model of care, responsible for delivering quality, clinical care for patients in managing their type 2 diabetes in a holistic and culturally appropriate way.

Key objectives will be achieved through effective and responsive chronic disease management, working in a collaborative multi-disciplinary team, including the Diabetes Educator and Aboriginal Health Worker (AHW).

This is a full-time fixed-term role of 12 months, with travel to Bundjalung Country required. The project is funded by MRFF funding from the Australian Government's Targeted Translation Research Accelerator program, delivered by MTPConnect.

About You

You will deliver technology-enabled, culturally appropriate care, including:

  • Remote patient monitoring
  • Evidence-based chronic disease management
  • Care coordination
  • Clinical escalation of concerning results, signs, or symptoms
  • Health coaching

Your Expertise and Qualifications

  • A demonstrated ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people and facilitate culturally appropriate group discussions
  • Registered Aboriginal Health Practitioner with unrestricted AHPRA registration
  • Clinical skills in chronic disease management and delivering care within a multi-disciplinary framework

  • Experience in working in virtual care
  • Experience in providing diabetes management
  • Living on Bundjalung Country

It is essential that the person is an Aboriginal and/or Torres Strait Islander person. This is a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991.
