- Conduct thorough health assessments to identify early warning signs of chronic disease.
- Collaborate with GPs to promote preventative healthcare strategies.
- Develop and implement initiatives to increase patient engagement and education.
- Manage administrative tasks related to CDM appointments efficiently.
- AHPRA registered Enrolled or Registered Nurse.
- Proven experience in Chronic Disease Management and health assessments.
- Strong organizational and time management skills.
- Excellent communication and customer service skills.
- A passion for people and a personable approach.
